A Family & Senior Assessment looks at the bigger picture. We review areas such as home safety, paperwork, medication organization, current family concerns, available resources and other details that may be easy to overlook. The goal is to identify priorities and give the family a clearer understanding of what needs attention now and what to prepare for next.
When an aging parent needs support, it can be difficult to know where to start. A Family & Senior Assessment helps identify areas that may need attention, including home safety, paperwork, medication organization, available resources and family concerns. Together, we identify priorities and create clear next steps for the family.
Every family situation is different. The assessment helps us look at the areas that may affect an aging parent's safety, organization and day-to-day well-being, while also considering the concerns of the family supporting them.
Home Safety & Fall Risks — potential hazards, lighting, bathroom safety and other concerns around the home
Medications & Organization — how medications and important health information are being organized
Important Paperwork — identifying and organizing documents the family may need access to
Daily Living Concerns — areas where the senior or family may be experiencing difficulty or uncertainty
Family Preparedness — what the family already has in place and what may still need attention
Resources & Referrals — identifying outside services or professionals that may be appropriate when additional help is needed
At the end of the assessment, the goal is to give the family a clearer picture of what is working, what may need attention and what the next steps could be.
The assessment looks at the practical details that can easily be overlooked when supporting an aging parent. Together, we identify concerns, organize priorities and determine practical next steps based on your family's situation.
